Instructions:

1. Download all forms- Application for Service, PA Child Abuse History Clearance Form, and Child Abuse History Consent Form
2. Fill them out completely
3. Have 2 references complete the reference forms
4. Place your finished forms in an envelope, seal it and address it to Tamara Brubaker, Coordinator of Volunteers

You may hand it in at the CMC (Children's Ministries Central) Desk in the west end of the lobby, or to the church office.  Thank you!
